In order to register for any of our programs, you must be a member by completing the application process. The process starts with the form below. Once your form has been received, we’ll reach out to you with any follow-up questions and schedule a get-together with existing members to ensure your family is a good fit for SDJH. Once your application is accepted, you will be required to pay membership dues in order to become a member and register for programs.
